Kết quả phẫu thuật Cox-Maze IV điều trị rung nhĩ kèm bệnh lý van tim tại Trung tâm Tim mạch, Bệnh viện E

Đặt vấn đề: Rung nhĩ là nguyên nhân chủ yếu gây đột quỵ, suy tim, đột tử và thường đi kèm với các tổn thương van tim. Kỹ thuật Cox-maze IV sử dụng sóng cao tần đơn cực và lưỡng cực thường phối hợp với các phẫu thuật van tim đặt biệt là van hai lá để điều trị rung nhĩ.


Phương pháp: Nghiên cứu cắt ngang, 40 bệnh nhân được điều trị rung nhĩ bằng phẫu thuật Cox-Maze kết hợp phẫu thuật van tim tại Trung tâm tim mạch Bệnh viện E từ tháng 01/2020-12/2021.


Kết quả: Trong 40 bệnh nhân, có 23 bệnh nhân nam (57.5%), 17 bệnh nhân nữ (42.5%), tuổi trung bình 53.6 ± 8.3 (36-68) tuổi. Thời gian phát hiện rung nhĩ trung bình 3.27 (1-9) năm. Có 5 bệnh nhân bị tai biến mạch máu não trước phẫu thuật chiếm 12.5%. Đường kính nhĩ trái trung bình của bệnh nhân là 49.2 ± 10.1 (21-70) mm, với tỷ lệ huyết khối tiểu nhĩ trái 8 bệnh nhân (20%). Tất cả bệnh nhân đều có bệnh lý hai lá kèm theo, tỷ lệ tổn thương van động mạch chủ là 4 bệnh nhân (10%), tỷ lệ hở ba lá nhiều kèm theo 13 bệnh nhân (32.5%). 100% bệnh nhân được thực hiện phẫu thuật Cox-Maze. Tỷ lệ cắt rung nhĩ và chuyển về nhịp xoang ngay sau mổ là 31 bệnh nhân (77.5%), có 2 bệnh nhân suy nút xoang (5%) và 4 bệnh nhân bị block nhĩ thất độ I-II (10%) nhưng đều không phải đặt máy tạo nhịp tạm thời, và hồi phục lại sau 1-3 tuần.


Kết luận: Phẫu thuật Cox-Maze IV cả bằng phương pháp đơn cực riêng lẻ lẫn kết hợp lưỡng cực đều mang lại hiệu quả và an toàn trên bệnh nhân rung nhĩ kết hợp phẫu thuật van tim đặc biệt là van hai lá.
