Factors associated with depression of hospitalized patients with reduced left ventricular ejection fraction heart failure in Hanoi Heart Hospital

Abstract

Objective: To survey the prevalance of depression and exproring factors associated with depression in hospitalized patients with  reduced left ventricular ejection fraction heart failure (HFrEF) in Hanoi Heart Hospital.


Methods: An analytical observational study was conducted in 178 in-patients with HFrEF at Hanoi Heart Hospital from June 2023 to August 2024. Data of medical history, clinical examination, echocardiography were collected. We use Beck Depression Inventory (BDI) to survey the prevalance of depression by direct interview patients.


Result: Prevalance of depression in study group was 15,8%, of which the majority were mild depression (63,9%), moderate depression was 32,3% and severe group was 3,8%. The women were likely to have depression compared to men (OR: 2,7 (1,2-6); p=0,01). The rate of depression was significantly higher in group of patients who live alone and were completely economically dependent. We also noted a higher rate of depression in sedentary patients, patients with 2 and more hospitalizations per year, and who have stayed at hospital more than 7 days compare to the other group. The group with high NT-ProBNP on admission (> 5000pg/ml) had more likely to have depression than the other group. There was no difference in prevalance of depression arcording to education level, residential area, age and degree of EF reduction.


Conclusion: The rate of depression in our study was 15,8%, which is lower than other study. Sedentary status, economic dependence, frequent hospitalization for heart failure, and prolonged hospital stay are factors that increase the risk of depression in this group of patients.
