Mitral valve replacement with full – sternotomy through a minimal skin incisions

Abstract

Objective: Minimally Invasive Heart Surgery is a common trend in the world. Using small skin incision with full – sternotomy in  mitral valve replacement surgery have not been reported in Vietnam. The study aimed to draw the initial experience in applying this procedure in Viet-Duc University Hospital. 


Methods: Retrospective descriptive study  in  42 patients who underwent surgery for mitral valve replacement with full – sternotomy through a minimal skin incisions, from 1 / 2010 to 8 / 2012. Results: including 29 female and 13 male, mean age  43.76  ±  10.46  (25  -  63).  Preoperative NYHA grade was not too severe (18 cases with I-II, 24 cases with III, there is no level IV) with left ventricular ejection fraction of 56, 93 ± 7.26% (43 - 74). Cardiopulmonary bypass time 103.93 ± 14.21 minutes (79-138). Aortic clamping time 80.36 ± 11.15 minutes (58-110).  Postoperative  mechanical  ventilation duration 15.64 ± 11.58 hours (5-48). Hospital stay 13.67  ±  5.78  days  (8  -  35).  No  complications  of sternal infection and postoperative bleeding. There is no death. Left ventricular ejection fraction after surgery: 64.71 ± 6.54% (52 - 75). Satisfaction of patients with small skin incision: 60% very satisfied, 40% satisfied. Conclusions: The mitral valve replacement with full – sternotomy through a minimal skin incisions is not a difficult technique, requiring no special equipment, enabling to  practise all  normal surgical times  with  high  security,  can  be  implemented  in routine cardiac surgery.
