Update indication for lung transplantation and practical application in Viet Duc hospital

Abstract

Overview: Lung transplantation (LTx) has been a new technology in Vietnam in recent years. Viet Duc univesity hospital was done 5/8 cases annouced in Vietnam. The objective of this article is to provide general information about LTx indications in recipients, applying to 5 LTx at Viet Duc hospital. Methods: Update literature-based review about LTx’s indication and retrospective description of 5 LTx from December 2018 to December 2020. Results: Approximately 80 chronic lung diseases with the indication for LTx in final stage, divided into 4 groups (A-obstructive pulmonary disease, B-pulmonary vascular disease, C-cystic fibrosis or immunodeficiency disorder, D-restrictive lung disease), the most common of which are chronic obstructive pulmonary disease, idiopathic interstitial pneumonia, cystic fibrosis, interstitial lung disease, idiopathic pulmonary fibrosis, pulmonary cystic fibrosis, pulmonary arterial hypertension,  alpha-1 antitrypsin deficiency emphysema. The prognosis after LTx was worst in the interstitial lung disease’s group. Out of 5 LTx cases performed at Viet Duc hospital: 3 in group D, 1 in group A and 1 in group B. There is 1 case with absolute contraindications, 2 cases with relative contraindications - clearly affect to the results after LTx. Good results in 2 cases, quite in 2 cases, and bad results in 1 case with absolute contraindications. Conclusion: Although it is the only solution that hopes to save lives of patients with some end-stage lung diseases, the appointment process should be very strict and adhere to recommendations, as it affects LTx results.
